Patient Connect Program

NFP’s Patient Connect Program provides solutions to medication and health insurance challenges faced by individuals with little to no income. A team of patient benefit specialists ensures that patients can afford their medications and are enrolled in programs to maintain long-term access.

Kidney Foundation of Ohio


Medication Assistance Program

The Medication Assistance Program provides the financial assistance needed for clients to access life-saving insulin, renal medications and nutritional supplements. These resources are critical for those living with renal disease and diabetes.

Cleveland Hearing & Speech Center


Hearing Aid Program

This program provides hearing aids and supportive services to improve the quality of life for clients who are under- or uninsured. The services are focused on those would not otherwise be able to afford hearing aids and the needed coordinating supports.

Centers for Families and Children


Medication Access (Circle Health Services)

This program increases access to medicine and medical supplies for un- and underserved populations. With six professionally staffed pharmacies, The Centers offers both traditional and innovative pharmacy services to promote health and wellness in clients served.

Care Alliance Health Center


Patient Assistance Program

Care Alliance focuses its services on individuals experiencing homelessness, residents of public housing and other underserved populations. This program works with patients outside the exam room to help them navigate the health care system, access the prescriptions needed and improve client health
