About
Collaborative Investments + Health brings together hospital networks, health insurers, and other stakeholders to impact the social determinants of health. It funds interventions supporting underlying health drivers, such as food and social interaction, to reduce downstream medical conditions and their costs.
This is a local implementation of a national model, the Collaborative Approach to Public Goods Investment (CAPGI). It aims to benefit both individual health and investor outcomes.
The current initiative is Nutrition Solution, which provides healthy, medically tailored food for lower-income individuals with chronic illnesses.
Offered through the Benjamin Rose Institute on Aging, this program supports participants through weekly wellness calls, nutrition education, and home-delivered meals.
Impact
During Cycle 1 of Nutrition Solution (April 2021-September 2022):
- Over 80,000 meals were delivered to participants
- Surveys showed participants had increased belief in the importance of healthy eating
- Evaluation revealed that improved nutrition was associated with financial savings for those with exacerbated health conditions and costly care
- Evaluation showed increased visits to primary care among participants
- Investors who shared cost and utilization data showed cost savings

Looking Ahead
The multi-stakeholder team – United Way of Greater Cleveland, investors, and the Benjamin Rose Institute on Aging – is currently finalizing Cycle 2 of Nutrition Solution. Learning from the successes and challenges of Cycle 1, we are fine-tuning aspects of the program design and evaluation to increase its benefit to both participants and investors.
